In interviews conducted between December 6 and December 9, respondents spoke by phone to a random national sample of 1,007 registered voters.
They asked Americans a series of questions about how they saw the President, who will be leaving the White House in January next year.
Around 8 percent of those questioned said that Mr. Trump will be remembered as "below average".
Almost a fifth (16 percent) said it would go down in the history books as "above average".
Meanwhile, 22 percent said the defeated incumbent was "one of the greatest" presidents the US has ever seen.
These numbers show how polarizing a character Mr Trump has become over the past four years is.
Despite losing the 2020 election to Joe Biden, who has now been elected president, Mr Trump still received more than 74 million votes.
His campaign team claims this is more votes than any other sitting president in US history.
According to Fox, the survey, which was carried out in collaboration with Beacon Research, had an error rate of +3 percentage points.
And while Mr Trump received nearly 11 million more votes in 2020 than in 2017, he has generated more negative sentiments than any other president, CNN reported.
The only other commander in chief who was so badly favored in a five-category poll at the end of his presidency was George W. Bush.
Around 36 percent of the voters polled by Gallup in a 2009 poll said that Mr. Bush would be remembered as a "poor president".
